At Prowers Medical Center, compassionate care goes beyond clinical treatment — and Nisha Longoria, Lead Case Manager, is a perfect example of how coordination and advocacy can truly transform a patient’s experience.

With 13 years of dedicated service, Nisha plays a vital role in ensuring that every patient receives the medical and social support they need — both during their hospital stay and beyond. As Lead Case Manager, she serves as the central point of contact for patient care coordination, collaborating closely with patients, families, providers, and the interdisciplinary team.

Her work includes conducting social needs screenings, facilitating transitions between hospital and home or other care facilities, coordinating swing bed services, securing insurance coverage, and ensuring compliance with healthcare regulations. She also supports hospital-wide quality improvement and patient safety initiatives.

“I truly enjoy being able to connect with our patients and help them in their time of need,” Nisha shares. “Knowing I can make a difference in someone’s life, especially during a difficult situation, provides me with great satisfaction.”

Nisha’s role is dynamic and challenging, but it’s her passion for service, and the supportive team environment at PMC, that keeps her energized.

Outside of work, Nisha enjoys spending time with her close-knit family and traveling to destinations that soothe her soul, especially the mountains and the ocean. She’s always seeking new opportunities to grow and learn.

Nisha holds a Master of Science degree with honors, and is a proud member of both the Omega Nu Lambda National Honor Society and the Order of the Sword and Shield National Honor Society. She also holds certifications in Health Navigation and Motivational Interviewing.

“Any opportunity I get to learn something new, I will take it,” she says — a mindset that continues to serve her, her team, and her patients well.
