Patient Medical Records

Patient medical records at Prowers Medical Center are managed by specially trained staff in our Health Information Management (HIM) Department. The HIM Department is located in the main Prowers Medical Center building at 401 Kendall Drive, Lamar, Colorado 81052.

Our Health Information Management Department is open to serve our patients Monday through Friday from: 8:00 a.m. to 5:00 p.m.

FEES FOR COPIES OF MEDICAL RECORDS

The Prowers Medical Center HIM Department uses the fee set by the State of Colorado for patient medical record reproduction.
